What to bring with you on the day of your Pick-Up
It is not always easy to adhere to administrative obligations during a fertility treatment, especially at the highest point of a sometimes difficult stimulation process. Nevertheless, it is extremely important that you remember to bring the following things with you on the day of Egg cell pick-up. It is a good idea to print out the list and keep it with your treatment plan.
What must you bring?
The form from the Mutuality is of crucial importance. If we do not receive this from you on the day of your Pick-up, the laboratory costs of your treatment will be billed to you. |
- Your identification label and your partner's.
- All documents that need to be signed in connection with your treatment, e.g. the consent for IVF or ICSI; the form about your decision regarding the surplus embryos; the form about your decision regarding scientific research; if you are taking part in the donation programme, the contracts pertaining to this.
- For the patients that fall under the reimbursement system of the Belgian health insurance: the consent form of the National Health Service, if we don't already have it.
