Persons coming to the CRG for fertility treatment are officially patients of UZ Brussel. There are a number of administrative aspects concerning this.
The information here is relevant for all patients in UZ Brussel. Only the section concerning the SIS- card is not relevant to overseas patients.
SIS-card
In Belgian Social Security, a card system is used for identity.
Since the introduction of the SIS (Social Information System) card in Belgium, you need to submit it with every medical consultation, be it with your doctor, your dentist, your pharmacist, your physiotherapist,
or in a hospital or outpatient clinic. Naturally, this also applies to every consultation and every exa-mination in UZ Brussel. This card guarantees the reimbursement of your medical expenses by the
NHS; this is why it has to be scanned regularly du-ring your treatment in one of our booths. During your fertility treatment at the CRG, we specifically ask you to do this at every consultation with the doctor, at every counselling session and when the first blood sample is taken at the start of the stimulation course.
Identification labelAs soon as you are registered as a patient at UZ Brussel, you will be issued with a white identification card or 'badge'. This badge refers only to you and is valid throughout the hospital, but only in this hospital alone.
If you change address, you will need to have your white badge updated.
You will be required to present this badge at all consultations and examinations without fail. It is used to minimize the amount of
paperwork and administration generated by a hospital appointment.
It is always possible to lose a card. Don't panic, we can make a copy in next to no time, at the admissions desk in the main entrance, or in the children's hospital.
Medical File
In Belgium, every hospital is obliged by law to make a file concerning every patient. These files are highly confidential and all members of hospital staff are bound by patient confidentiality: no information from these files may ever be discussed unless within the hospital and specifically concerning your treatment.
The creation and storage of medical files like with every other medical document is done in accordance to the law of 8 december 1992, which concerns the protection of personal data.
Every time you attend the CRG, a report is produced and added to your file. We act in accordance of the law passed upon 22 august 2002, concerning patients' rights.
A copy is made of every report written, which is the sent to your GP (and gynaecologist). During your initial visit, you will be asked to provide the name and address of your GP, in writing. After treatment, a medical report will be sent to him or her a few weeks later.
If details concerning your GP change during the course of your treatment, do not forget to inform us of the changes.
This also applies to changes in your personal situation (moving house, separation from partner, spontaneous pregnancy ...). We would like to be immediately informed of such changes.
