Practical Information


When coming to the CRG for a treatment for the first time, you will be asked to fill in a questionnaire regarding your medical and family history. You can download the questionnaire here, print it and fill it out in advance: questionnaire woman, questionnaire man.

The CRG regularly organizes information evenings (in Dutch and in French) for everyone who would like further information regarding fertility treatment.
 
Patients undergoing treatment at the CRG are officially patients of UZ Brussel. See administration for information regarding administrative formalities, plus the use of your SIS-card and white identity card, and over the maintenance of your medical file.
 
Using the following links, you can obtain information regarding the practical aspects of examinations/interventions or practical aspects of treatment in order to know where you need to be for various investigations or examinations. Click on the link for information specific for overseas patients. From there you will be able to click through to other areas of the website, so that you find information in the correct order.
 
Financial information can be found by clicking on the link. There is also financial information for Belgian patients (in connection with the Belgian Mutuality) and overseas patients.
 
If you undergo fertility treatment, you will be required to complete and sign a number of contracts and consent forms. Under documents you will find downloadable examples of all the information sheets, contracts and instructions which the CRG issues to its patients.
